Description
Female to Male surgery in Thailand involved removing the uterus and ovaries of the patient and altering the external genitalia to resemble those of a male. The urethra is lengthened and once the vaginal opening has been closed a phalloplasty is performed, fabricating a realistic male sexual organ.
Who is a candidate for female to male surgery in Thailand?
A healthy individual above the age of 20 who suffers with gender disorder is suitable for this type of surgery. Prerequisites are that they undergo a year of hormone therapy, psychological counselling and that they spend a year living as a male before considering the surgery.
How is the surgery performed?
Metoidioplasty
Dr. Preecha is performing Dr. Miroslav's method of meta or metoidioplasty whereas the position of the enlarged clitoris is released and moved forward to become more close to the position of a normal penis. In some cases the doctor will apply a urethral lengthing if possible which is included in the price. The urethra will be lengthened to end at the tip of the neophallus. The vulva (labia majora) can be united to form a scrotum, that is where the prosthetic testicle implants (silicone) can be put. This procedure is much simpler than phalloplasty and much less complications.
The result of this procedure unlike a normal phalloplasty an erectile prosthesis is not needed to achieve erection and sexual arousal, but still this will depend on the size of the clitoris which differs from person to person.
Phalloplasty
There are several stages to this procedure. First, a subcutaneous mastectomy is performed, a procedure which takes about 2 hours and will remove any breast tissue, giving a masculine appearance to the chest area and will require a 7-9 days recovery in the hospital.
The removal of the uterus and ovaries is imperative to stop menstruation and can be done together with the mastectomy or as a separate procedure. After a three month period the patient will need to go for a consultation to ascertain whether the wounds have healed appropriately and to decide whether the patient is ready for the next phase.
The vaginal closure and urethral lengthening is the next step in the process and will require a week long stay in the hospital. The vagina is closed and the urinary tract is lengthened and the vaginal walls are folded outwards, making urination towards the front possible. The patient is required to stay in hospital for 2 days and wait 2 weeks for subsequent healing before returning to complete the final stage, which is the fabrication of the penis.
A flap of skin grown on the forearm is used in urethral prefabrication. Microsurgery is used to transplant the skin, nerves and blood vessels and a catheter is wrapped in glutei skin and is buried in the forearm to minimize urine leakage while the healing process takes place. The result after healing is a functional urethra. A 2 night hospital stay is required and you will have a catheter in your arm (used to form the urethra) and will need regular checkups during the three month healing process before you are ready for the next phase.
The final stage is the creation of the actual penis from the forearm flap. This is transferred to the genital area and all the blood vessels and nerves are connected using microsurgery. The surgeon creates a penis that is proportionate to the length of the foreskin, usually 4 – 6 inches. A scrotal sac is created from the labial skin and fatty tissue, without testes.

4th Stage: This stage should be done after 3 months from the last stage, and it includes the insertion of the silicon penis to make it hard and sculpture the gland penis + minor corrections. This stage requires one night at the hospital and 7 days in Thailand.

All patients recover quickly from the surgery because it is done in distinctive stages to avoid complications. After the procedure the patient will have a fully functional penis through which they can urinate and which has all the physical characteristics of a normal penis.
Post care
Patients must return for removal of stitches after two weeks and should not engage in sexual activity for two months after surgery. The surgeon will explain everything you need to know during the recovery period.
