A small incision is made along the inside of the cheek, near the gum line above the second upper molar.  The surgeon applies external pressure to the cheek muscle, under the cheek bone, which forced the cheek or buccal fat to protrude through the incision.  The excess fat is drawn out by means of a forceps and then trimmed away until the surgeon is satisfied that enough has been removed to create a more sculpted look to the cheek area.  The incision is then sutured closed.
